介護従事者の半数以上が、要介護認定の必要性を認める一方、制度そのものは簡素化すべきと考えていることが、淑徳大の結城康博准教授らの調査で分かった。また、介護保険制度の運用の在り方については、全国一律のサービスを担保するため、地方分権を推進するより、厚生労働省がある程度の権限を維持すべきと考える介護従事者が、半数近くに達することも明らかになった。

結城准教授と本多敏明助手らは、特に高齢者人口の急増が予測される首都圏を中心に、1都7県（宮城、栃木、埼玉、千葉、東京、神奈川、滋賀、兵庫）のケアマネジャーやホームヘルパーらに質問用紙を手渡し、回答を記入してもらう手法による調査を実施。昨年5月から12月にかけて723人分の有効回答を得た。

要介護認定についての質問では、「要介護認定システムは簡素化する必要はあるが、必ず必要である」と回答した介護従事者は56.8％と半数以上に達し、「現状維持でよい」（13.6％）を大きく上回った。また、「ケアマネジメントが担保されれば、（要介護認定は）必要ではない」との回答も11.8％に達した。「わからない」は17.8％だった。結城准教授は「今後は現場の従事者が要介護認定システムの何を簡素化してほしいと考えているのか、調査・分析に力を注ぎたい」と話している。

　地方分権と介護保険についての質問では、「ある程度厚労省が権限を持ち、全国画一的なシステムを担保すべきである」との回答が48.8％と半数近くに達した一方、「介護保険は地方分権に基づいて自治体に権限を委譲すべきである」と「わからない」は共に25.6％となった。結城准教授は「介護の現場では、地方分権の推進だけでなく、介護サービスの質の担保をナショナルミニマムとして求める傾向もあるのではないか」としている。

■現金給付のシステム化を求める声も

　そのほか介護保険の家族介護における現金給付についての質問では、「現物給付に特化し、現金給付はシステム化すべきでない」との回答が30.4％で最も多かった。ただ、「ドイツのように、一部の家族介護に対し、現金給付をシステム化すべき」は20.9％、「介護サービスが整っていない地域に限定して、一部現金給付をシステム化すべき」は18.9％で、何らかの形で現金給付を求める意見は39.8％となり、「システム化すべきでない」とする意見を上回った。「わからない」は29.7％だった
