介護報酬改定で現場は変わったか？

「介護報酬改定後、現場はどうなった？」と題した情報交換会では、参加した介護従事者が「居宅介護支援」「訪問介護」「通所介護」のグループに分かれて、今年度の報酬改定で現場にどのような影響があったかなどを話し合った。後半の報告会では、各グループが報告を行った。
　居宅介護支援のグループからは、給与を上げた事業者と様子見の事業者があるという話が出たほか、今年度の補正予算に盛り込まれた「介護職員処遇改善交付金」の対象にケアマネジャーが入っていないことへの不満もあった。今年度の改定で新設された「独居高齢者加算」について、比較的容易に取得できる自治体がある一方で、住民票の提出などを求める自治体もあり、対応が異なるという指摘もあった。
　訪問介護のグループからは、介護報酬がアップしても利用支給額は上がらないことから、サービス利用が減るといった影響が出たり、加算が取れる場合でも、事業所が利用者の負担を考えて取得を躊躇したりするという指摘があった。このほか、「リフレッシュ手当」という名目で、処遇改善のため月額3000円を支給する事業所もあるという。
　また、事業所はヘルパーに休日手当を出しているが、休日に対応する加算がないことに疑問の声があったほか、今年度の改定でサービス提供責任者による初回と緊急時の対応への加算が新設されたが、仕事がハードであるにもかかわらず報酬が伴っていないという意見もあった。
　通所介護のグループからは、事業所の運営主体が社会福祉法人と株式会社の場合では、賃金についての考え方が違うという指摘や、送迎についての加算を設けてほしい、複数人で介助する入浴と機械浴の報酬が同額なのはおかしいなどの意見があった。
