たまゆらを繰り返すな－。東京都台東区のＮＰＯ法人が、既存住宅に医療や介護の利用援助を付加し、「支援付き住宅」として活用する方法を提唱している。今年３月に起きた群馬県の無届け施設「静養ホームたまゆら」の火災では、施設不足を理由とする生活保護費の越境支給が明らかになり、「住宅政策の問題」との専門家の指摘もある。再び犠牲者を出さないためには、独居の困難な低所得高齢者の住宅確保が急務だ。（寺田理恵）

　◆自分のペースで

　墨田区の木造住宅が密集する地域に今年５月、「支援付き住宅ふるさと晃荘」がオープンした。たまゆらなど都外の無届け施設から戻った生活保護受給者６人を含む１８人が暮らす。

　老朽化で借り手のいなくなった木造アパートを、ＮＰＯ法人「自立支援センターふるさとの会」が借りる前提で家主が建て替えた。食費込みの利用料は生活保護費で賄える約１４万円。常駐職員が医療・介護の手配や生活上の手助けを行う。

　個室は３畳と狭いが、病院を転々とする社会的入院を続けてきた男性（６１）は「自分（のペース）で生活できる」。ほかの施設から移ってきた男性（７３）は「気を使わずに済む。以前は４人部屋で、外出もできなかった」と、生活が落ち着いた様子だ。

　同会の滝脇憲理事は「脳梗塞（こうそく）で在宅が困難になり、同じ地域の自宅から晃荘に住み替えた事例もある。高齢者の住宅不足は生活保護だけの問題ではない。晃荘は民間資金で住宅を確保するモデルになる」と話す。

　◆政策の乖離が問題

　同会は東京・山谷で社会的入院患者の受け入れ施設を運営するが、退院後も医療や介護の必要な高齢者の緊急入居が急増。受け皿確保が難しいのが実情だ。そこで支援付き住宅の制度化を提唱し、研究者や医師らと研究会（研究委員長・山岡義典法政大教授）を立ち上げた。福祉関係者が集まった８月の推進会議では「空き屋率は高いのに２４時間ケアができる住宅ストックがない」「公営住宅の空き室を使えば解決するはず」など、福祉と住宅政策の乖離（かいり）が指摘された。

　国土交通省の調べでは、介護保険の要支援・要介護認定者４５０万人に対し、介護・生活支援付きの住まいは定員１３０万人。たまゆらでは無届け施設に対する行政の監視不十分が問われたが、根底には高齢者の施設や住宅の不足がある。

　早川和男神戸大名誉教授（建築学）は「たまゆらの火災では生活保護を食い物にする貧困ビジネスが批判されたが、問われるべきは国の住宅政策。たまゆらも、７月に山口県の特別養護老人ホーム入所者が土石流の犠牲になった災害も一見、福祉の問題として表れたが、辺鄙（へんぴ）な場所や危険な地域に追いやるのは住宅政策に問題があるからだ」と話している。

　■「社会的入院減り行き場なく」

　入院治療の必要がなくても入院を継続する社会的入院は、減っているとみられる。医療費を削減するため、入院日数が長くなると病院収入が減る仕組みが１０年ほど前から導入され、病院を転々とする転院問題が生じた。たまゆら火災を契機に、山梨県の無届け施設から晃荘に移った７０代男性は転院を繰り返す間に足腰が弱って歩けなくなったという。現在はさらに短縮化が進み、退院しても家庭の事情などで自宅に戻れない高齢者は少なくない。受け皿整備の遅れもあり、退院の促進が行き場に困る高齢者を増やす原因の一つと指摘されている。 産経新聞
